REQUIRED SOMN FORMS:
In addition to filling out a South Region Stars sport specific form, athletes MUST also be registered with Special Olympics of Minnesota. Below are a series of forms that must be completed; these can be returned directly to the SOMN office or can be turned in to Marcie Padgett.

Application for Participation: Medical and Consent Form. This link contains all the forms necessary for an athlete to participate! (This form expires three years from the date of the athlete's physical.)

Healthy Athletes Release. This completed form allows athletes to participate in any of SOMN's health screenings.

Athlete Consent Form. Only the adult version of the consent form, for those over 18

Unified Partner Application.This form is only needed for Unified Sports partners. Partners are athletes who do not have a disability, but individuals who can train and compete in certain sports as particpants alongside our athletes.
